Unique system introduced by the Zorgverzekeringswet (Zvw, 2006): every resident must purchase basic health insurance from private insurers. Government defines and subsidizes the basic package (basispakket). Supplementary insurance (aanvullende verzekering) is optional. Premiums are community-rated (same premium regardless of age or health status). Income-dependent contribution (Zvw-premie) paid via payroll tax funds risk equalization between insurers.
